On a PS 5, After returning to the ship after finding the under ground bunker in the Viper's Nest mission the campaign is no longer in the menu of active missions. The Journal is is still on Viper's Nest with no check mark on "Find the Alpha Legion Vox Array on  Vismar Minoris.  I used a different email in the game but never got a confirmation email so I had to create this account to file this report. My PSN Name is Vedius. I will be happy to share my email, just not publicly. 

I have three Inquisitors and this is happening with the Assassin I have created. Did not have this problem with the earlier run with the Crusader.
