I just started playing and took on this mission to eradicate the warp gates of chaos and suddenly i disappeared into the floor, as you can see on the picture i send, it only happend on these corner walkways (the triangle im standing in on pictures). 
(sry for bad resoulution and bad english).

I've noticed this same issue. It also happens occasionally while using the assault armor jump attack (in the same terrain setting).
