Seems like the aetherwalker armor on psyker is overriding the skull helmet if you have it enabled.

how do i claim it? i did the event but nothing showed up. no reward. something i miss?

Thank you for the report! That's a known bug, and as far as I know, we have already fixed that in the internal build. Patch is coming soon!
