Just equipped a new one handed weapon. Entered a new mission and it was gone. Exited mission, game won't let me equip into the empty slot.

Hmm, odd indeed. Must be randomly created on character upon creation which then proceeds throughout the created character...

Nope, it was a psyker.

Let me guess:

Your primary class is Assassin? I've tried both Crusader & Psyker, but their weapons stay normal, unlike Assassins, their weapons reset to default and deletes newly equiped weapons.
